You will need to bring the following items to register a new student:
Copy of the child's Social Security Card
Copy of the child's Birth Certificate
2 proofs of residence such as: Signed Lease, Deed, Mortgage or Letter from Property Owner, Utility Bill or Phone Bill.
MUST HAVE AN ORIGINAL (Not Copied or Faxed)Immunization Form (blue) issued by the State of Alabama. You may have these forms issued at either the Madison County Health Department or your family physician.
To enter Kindergarten, your child must be 5 years old on or before September 2.
To enter Grade 1, your child must be 6 years old on or before September 2.
