Things you will need to enroll a new student.
The student's Birth Certificate (a copy will be made and the original will be returned)
The student's Social Security Card ( a copy will be made and the original will be returned)
Immunization (Blue) Form issued by the State of Alabama. (original NO copy) Immunization forms may be obtained from your physician or the Madison County Health Department.
2(two) Proofs of Residency, with current address such as: (utility bill, phone bill, lease agreement, or deed )
Please Note: A student must be 5 years of age, on or before September 2nd to enroll in the Huntsville City Schools' Kindergarten Program.
